Purpose:
The person in this position will provide direct casework and counseling services to the hospice patient/family as directed by the plan of care. The social worker provides psychosocial assessments and develops a corresponding care plan as coordinated with the Interdisciplinary Team
Job Description:
- Provide psychosocial assessments, direct casework, and counseling services to patients and their families.
- Participate in the preparation, evaluation, and execution of the interdisciplinary care plan to assure that the aims of continuity and comprehensiveness of care are accomplished.
- Work collaboratively as a member of the Interdisciplinary Team(s).
- Prepare clinical and progress notes for the clinical record per Hospice, state, and federal guidelines/standards.
- Prepare documentation for the clinical record per Hospice, state, and federal guidelines and standards.
- Maintain and update knowledge of community resources. Facilitate referral of patients/families to those resources as needed and desired.
- Participate in taking on-call responsibilities as necessary, including weekends and evenings.
- Participate in the Interdisciplinary Team meetings, staff meetings, and other meetings as requested.
Qualifications:
- A Master's degree from an accredited educational institution of social work is preferred, or another related clinical degree.
- Must be a licensed social worker
- Strong interpersonal and communication skills are required
- Experience in health care and clinical social work is preferred
- Knowledge of the process of death, dying, and bereavement is required
- Understanding of psychosocial factors operative in terminally ill patients and families required
- Unlimited access to reliable transportation
